| WVU Medical/Dental Records | Data from a WVU Medicine medical or dental record must be requested from WV
CTSI. Requests are reviewed and approved by
WVCTSI. You may be contacted for questions related to the request, and
additional request forms and HIPAA documentation such as a
HIPAA Waiver of Authorization may be required.
You will be prompted to select if your request is for a Fully - Deidentified Data Set, a Limited Data Set, or a Data Set with Identifiers. Fully-De-identified data sets provided from WV CTSI do not require a HIPAA Waiver. Limited Data Sets and Data Sets with identifiers do require a HIPAA Waiver. |
| Approved HSC Data/Biospecimen Repository | An approved repository stored on HSC/WVU Medicine managed facilities or data
storage. HSC repositories are approved by WVU Medicine/HSC staff and
WV CTSI provides facilitation of approvals.
If you are unsure if a repository is approved, select OTHER WVU DATA SOURCE and indicate the source. You will be prompted to select if your request is for a Fully - Deidentified Data Set, a Limited Data Set, or a Data Set with Identifiers. |
| Other WVU Data Source | Any internal secondary data source (the data was created by WVU). The
data may be identifiable or not. Do not select this source for WVU
medical/dental records, HSC repositories or WVU Library data sets.

| External Data Source | Any external secondary data source provided to the research team. The data
may be identifiable or not.
|
| Public Data or Data Set | A data set from the WVU Library or other publicly available internal or external
source. This data can also contain identifiers available to the general
public. (e.g. a phone book)

| Data Collection - Anonymous | The data will be collected so that responses cannot be tied to a participant
by anyone (e.g., survey, interview, interaction, intervention).
NOTE: Anonymous does
NOT mean
de-identified, coded, or anonymized.

| Data Collection - Identifiable | The data will be collected by the researcher directly from the main source
with
identifiers. At least one variable from the list should be selected.
