Alteration of Consent

Short Form Consent

Short form consent is an alternative to using a translated consent form. The form contains a description of the required elements of informed consent and notes that the information will be presented orally to the prospective participant or legally authorized representative.


The WVU IRB must approve the short form and the written summary (if applicable).

A witness to the oral presentation is required.

The short form must be signed by the participant or legally authorized representative.

The witness must sign the short form and the written summary (if applicable).

A copy of the short form and the written summary (if applicable) must be given the the participant or the legally authorized representative.
